Actually “Salus extra ecclesiam non est” was introduced in the 3rd century by St Cyprian in the context of rebaptizing those baptized by heretics. While others modified the concept, Cyprian’s opinion on the baptism matter was refuted by Augustine. So it’s not an “always,” thing.
